Please Note: Child must be 5 years old on or before December 1st of the school year they enter Kindergarten.

1. Please complete the registration packet which can be downloaded by clicking here: Registration Packet

2.  PRINT the following forms to be completed by the appropriate health care professional: 

• Physician Report (K-12)
• Dental Health Certificate
   

3.  Please provide us with the required documentation which can be downloaded by clicking here:  Required Documentation. 
 
 4. Documents will be accepted via US Postal Mail, fax, email, or by appointment.
