Attention Parents/Guardians of Incoming 6th graders

All students entering 6th grade in New York State are required to be vaccinated against Tetanus, Diphtheria and Acellular Pertussis (Tdap). Please be sure that your soon-to-be 6th grader has this immunization before school starts in September 2019. Proof of this immunization must be provided to the Clinic/Health Office prior to September 5 or your child will not be able to attend school. Please call your child's doctor to make sure that he/she has had this immunization.  Proof of Tdap can be faxed to the school at 315-785-3769 or dropped off to the Main Office during the summer (Monday - Friday between 8:00 am - 3:00 pm).

Jefferson County Public Health Department offers the Tdap vaccines every Wednesday from 12:30 to 3:30 pm at 531 Meade St., Watertown, NY.  For more information call Public Health at (315) 786-3770 or go online to make an appointment at .
